This is a Gould & Goodrich K381-U45 Double Retention Duty Holster for an H&K USP 9, 40 or 45. This is from the K-force line. Item is new. I have several of these from a defunct police supply store. They are all new but do not appear to be in the original Gould & Goodrich bags.  They do have the instruction sheet with them.  Right Handed design.
